Glaucoma surgery

Glaucoma surgery

Glaucoma surgery operates on the smallest structures of the anterior segment of the eye. In a trabeculectomy, you create an artificial drainage pathway for the aqueous humour. MIGS procedures, the minimally invasive glaucoma surgeries, are even more delicate: You place tiny stents or shunts, often only a few millimetres in size.

The angle of the anterior chamber is difficult to view, and the working distance is minimal. The microscope must provide you with maximum illumination along with high magnification. Depth of field is crucial because you work on different planes. From the conjunctival flap to the trabecular zone, there are several millimetres of depth difference, and everything must be depicted sharply.

Coaxial Lighting for Chamber Angle Procedures

The coaxial arrangement of light and observation beam is crucial in glaucoma surgery. The chamber angle is deep and difficult to observe. Traditional lighting systems cast shadows that obscure the finest structures. The coaxial cold light fibre optics or LED lighting on Karl Kaps microscopes provides shadow-free illumination, even in hard-to-reach areas.

The retina protection filter GG 475 safeguards the eye, even when the lighting is used for an extended period. This is particularly important during trabeculectomies, which can last up to 45 minutes. UV and IR filters are part of the standard equipment and reliably filter out harmful wavelengths.

Motorised Zoom for MIGS

Minimally invasive glaucoma surgery requires stepless magnification. The tiny stents and shunts are hardly visible at low magnification. You zoom in and out during the operation without taking your eyes off the chamber angle. The footswitch control makes this intuitive and quick.

During iStent implantation, maximum magnification is needed to locate the precise placement point within the trabecular meshwork. You zoom out for an overall view. During canaloplasty, you switch between different magnification levels depending on whether you are inserting the catheter or checking its position.

XY Coupling for Angled Viewing

In chamber angle procedures, you often view the eye from an angle. The motorised XY coupling positions the microscope head precisely over the optimal viewing angle. This is particularly helpful in challenging anatomical situations. The speed control allows you to adjust movement speed. You move slowly for fine adjustments in the chamber angle, increasing speed for broader movements.

Shadow-Free Illumination of the Chamber Angle

The coaxial illumination provides shadow-free lighting of the chamber angle. Even in deep and hard-to-reach areas, you can see every detail. The finest trabecular structures become visible, and the placement of micro-stents can be controlled precisely.

Seamless Zoom for Tiny Structures

The motorised zoom with a 1:6 ratio enables seamless adjustment from overview to maximum detail magnification. During iStent implantations, you can zoom in on the exact placement point within the trabecular meshwork.

Optimal Viewing Angle with Swivel Tube

The swivel tube from 0 to 210° can be adjusted so you have an optimal view into the chamber angle. In chamber angle procedures, every degree counts. Each surgeon can find their optimal working angle.

Memory for Various MIGS Procedures

Every MIGS procedure has specific requirements for magnification and illumination. The memory function saves five configurations. With a single button press, all parameters adjust for the respective procedure.

Extended XY Coupling for Oblique Viewing

The 62x62mm range of movement of the SOM4OPHT provides more flexibility for oblique viewing angles. Especially in chamber angle procedures, where optimal viewing is often only possible from a certain angle, this makes a difference.

SOM 62 Ophthal Advanced

Ideal for minimally invasive glaucoma surgery. Motorised zoom at a ratio of 1:6 gives you seamless magnification for MIGS procedures. The inclinable binocular from 0 to 210° allows you to adjust the angle for an optimal view into the eye. Every degree counts when working with the anterior chamber angle.

The motorised XY coupling with a 50x50mm movement range precisely positions the microscope over the anterior chamber angle. The foot control panel manages zoom, focus, XY coupling, and light intensity. The centre button automatically returns the microscope head to the central position, which is practical when changing patients.

The coaxial cold light fibre optics with 15V/150W illuminates the anterior chamber angle without shadows. The retina protection filter GG 475 shields the eye during longer procedures. The motorised fine focusing with a 40mm range enables precise focusing on different depth levels.

SOM4OPHT

The flagship model for complex glaucoma cases. The separate LED red reflex illumination gives you additional control over the lighting. You can adjust the intensity of both light sources independently and find the optimal combination for each situation. The memory function stores five different settings. Trabeculectomy, iStent implantation, canaloplasty – each procedure has its own preset button.

The expanded XY range of 62x62mm provides more flexibility in positioning. Especially for chamber angle work, where you often look at an angle into the eye, this is a great help. Speed control allows you to individually adjust the movement speed. The advanced filter options (green filter, orange filter, blue filter) enhance the contrast under challenging visibility conditions.

Technical specifications:

  • Magnification: Motorised zoom 1:6
